Responsibility for Providing Care and Supervision — Based on LPAs interviews and record review, staff did not comply with the section cited above as a child was left behind on 2/21/25 in a classroom & 3/6/25 unsupervised in the outdoor playarea, which posed an immediate threat to the health and safety of the children in care.
