Responsibility for Providing Care and Supervision — Based on record review and interviews Child was seen left unattended on the patio between classroom 5 and 6, which poses an immediate health, safety or personal rights risk to persons in care.

Personal Rights — Based on interviews the licensee did not comply with the section cited above by ensuring staff are not handling day care children inappropriately, which poses an immediate risk.

Interviews revealed C1 was left without supervision on multiple occassions, which poses an immediate risk to the health and safety of children in care.

Reporting Requirements — Staff interviews and record reviews conducted during the course of the investigation, it was determined that the center failed to report an incident that occurred sometime in December 2025.
