Personal Rights — Based on interviews, video footage, and record review, it was found that S5 inappropriately spoke to C1 by yelling as well as handled them roughly. This posed an immediate health and safety risk to children in care.

Responsibility for Providing Care and Supervision — Based on observations made by LPA Placencia as well as LPA Medrano and Campos of children being left in classrooms unsupervised by an adult. This poses an immediate health and safety risk.

Responsibility for Providing Care and Supervision — Based on Staff and Parent interviews conducted. Staff and Parents indicated that there were two children that were left behind at Chuck E Cheese

Personal Rights — Based on observations made by LPAs, a strong, concerning, unknnown odor was detected being present inside of the facility. Due to the unknown and strong nature of the odor, it is determined that this is an immediate risk to the health, safety, or personal rights to children in care.

Drinking water lead testing — LPAs observed two (2) aides supervising nine (9) toddlers without the direct supervision of a fully qualified teacher. Facility was previously cited twice for the same citation. This now poses an immediate risk to the health and safety of the children in care.

Personal Rights — evidence was obtained showing children were present during a flood where children were exposed to unsanitary standing water inside of the facility. This provides an immediate risk to the health and safety of the children in care.

Food Service — Based on observations, and interviews the facility did not accomodate children's request for additional servings. This poses a potential health and safety risk to children in care.

Teacher-Child Ratio — Based on observations made 8/6/2025 the facility was out of compliance with having S5 and S6 alone with 27 children while other staff were outside of classrooms. This poses a potential health and safety risk to children in care.

Limitations on Capacity and Ambulatory Status — Based on observation and interviews, the licensee allowed children who were identified as being enrolled at another licensed facility to be present at this facility. This poses a potential health and safety risk to children in care.

Preschool Program with Toddler Component — Based on interviews and documentation received it was found that the facility will commingle the preschool program with the toddler program when they are "short staffed." This poses a potential health and saftey risk to children in care.

Reporting Requirements — Based on interviews and physical evidence received C1 sustained an injury resulting in hair loss that was not reported by the facility. This poses a potential health and safety risk to children in care.

Revocation or Suspension of License — information obtained from interviews, indicating that based on interviews, record reviews, and observations the facility did not comply with section cited above as the owner has given information and direction to staff that contradicts Licensing regulations, which poses a potential risk to the health and safety of children in care.

Reporting Requirements — Facility staff did not send the Department an Unusual Incident report for the flooding that occured on 2/22/2024. This poses a potential risk to the health and safety and/or personal rights to the children in care.

Personal Rights — Through LPAs investigation it was found that child(ren) in care had been hit by facility staff. This presents a potential risk to the health and safety of the children in care.

Personnel Requirements — , through LPAs investigation it was found that facility staff were required to work at the facility with the children when they were ill. This presents a potential risk to the health and saftey of the children in care.

Teacher-Child Ratio — During today's visit, LPAs observed two aides supervising 15 children alone without the direct supervision of a fully qualified teacher which poses a potential risk to the health, safety, or personal rights to children in care.

Teacher-Child Ratio — During today's visit, LPAs observed two aides supervising 15 children alone without the direct supervision of a fully qualified teacher on a walking field trip away from the center which poses a potential risk to the health, safety, or personal rigths to children in care.

Personal Rights — LPA's observed facility staff speaking to the children in care with inappropriate words and a harsh tones. This presents a potential risk to the health and safety of the children in care.

Teacher Aide Qualifications and Duties — Through record review and observation it was found that 3 staff members were supervising children alone without direct supervision of a teacher. Staff does not have record of transcript or permit on file verifying the ability to be left alone with the children in care. This presents a potential risk to the health and safety of the children in care.

Teacher-Child Ratio — During the visit, LPAs observed one teacher's assistant left alone in a classroom with 13 children which poses a potential risk to the health, safety, or personal rights to children in care.

Personnel Requirements — During record review, LPAs Uribe & Pringle found that some staff members did not have a complete Health Screening Report (LIC 503) form in their personnel file which poses a potential risk to the health, safety, or personal rights to children in care.
