Buildings and Grounds — both staff bathrooms were not in good repair, which posed a potential risk to the health and safety of the employees present.

Teacher Aide Qualifications and Duties — A teachers aide was left alone by their co-teacher with children on multiple occassions which poses a potential risk to the health and saftey of the children in care.
