Personal Rights — Based on interviews, it was determined that Staff #2 spoke inappropriately to children and held Child #1 while instructing Child #2 and Child #3 to hit Child #1 back.
The date by which the licensee must file CCLD's Proof of Correction form, certifying under penalty of perjury that the deficiency was corrected on or before that date. Filing it does not prevent CCLD from making a follow-up visit to verify.
No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Based upon staff interview, Staff left a child unsupervised in the restroom and did not realize it until a child brought it to staff attention. Due to lack of supervision this poses as an immediate risk to the health and safety of the children in care.

The child care center of any of the events specified in (d)(1) below, report shall be made to the Department by telephone or fax within the Departments next working day and during its normal business hours…Any injury to any child that requires medical treatment. Based upon staff interview, staff did not report of injury to child within the Department’s next working day and during its normal business hours. Due to lack of Reporting Requirement this poses as a potential risk to the health and safety of the children in care.

Buildings and Grounds: (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. During today's inspection of the play yard, LPAs observed the fence by classroom 4 to have two unraveled parts that have a sharp point poking out. LPAs observed holes where the sprinklers are located, the privacy slats on the fencing around the air conditioner was coming unraveled and laying on the ground, there were mushrooms growing from the ground near one of the play structures, and paint chipping on the exterior wall. In room 10, ceiling panels show water damage. — requirement not met
