Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that four staff members were missing immunization records (Measles/Pertussis), and two staff members were missing Tuberculosis (TB) test results in their files.
The date by which the licensee must file CCLD's Proof of Correction form, certifying under penalty of perjury that the deficiency was corrected on or before that date. Filing it does not prevent CCLD from making a follow-up visit to verify.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that four staff members were missing Health Screening reports.

During the indoor inspection of both classrooms, Licensing Program Analyst (LPA) observed that the rugs used for circle time had dark stains and appeared not have been shampooed for some time. The blue carpet in both classrooms also appeared stained in some areas.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During employee record review, Licensing Program Analyst (LPA) observed that three of the new staff did not have proof of completed Mandated Reporter Training for Child Care Providers on file.

101223(a)(3) Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights:(3) To be free from corporal or unusual punishment, infliction of pain . . . including eating, sleeping or toileting; or withholding of shelter, clothing, medications or aids to physical functioning. This requirement was not met as evidenced through interviews with staff, which indicated that a contracted employee from an outside agency aggressively handled a child during classroom hours. This poses a potential risk to the health, safety, and personal rights of children in care.
