Plan of Operation — Center did not notify parents/guardians and the elementary school office of not picking up a child from school, as per the center's plan of operation under the center's transportation contract, which poses/posed a potential health, safety or personal rights risk to persons in care.

Indoor Activity Space for School-Age Children — LPA observed 2 school-age children inside the preschool classroom. LPA reviewed with Director, Sandra Graham that children can not be commingled, which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observed, the licensee did not comply with the section cited above in that LPA observed a community sandbox that is used by both school age and preschool ages. LPA observed that sandbox had multiple animal fecal inside. LPA observed that sandbox did have a cover, yet sandbox was not covered when not in used. LPA discussed with Staff #5 best practices, which poses/posed a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on record review and interview, the licensee did not comply with the section cited above in that LPA was informed that Staff #5 who is a qualified teacher supervises both Preschool and School age students by standing in the middle of both classes. LPA was informed that Teacher aides do help out, but that the facility does not have a qualified school age teacher, which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in During record review LPA observed that Staff #6 did not have the required immunization's on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA also observed during record review that there is no LIC308 for Staff #5 or any of the staff present on today's inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.
