Buildings and Grounds — Based on LPA observation and Licensee interview, LPA observed an Ajax Bleach Powder can located in accessible PS children bathroom next to the sink faucet. Licensee stated it must had been left out by the cleaners this morning. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
The date by which the licensee must file CCLD's Proof of Correction form, certifying under penalty of perjury that the deficiency was corrected on or before that date. Filing it does not prevent CCLD from making a follow-up visit to verify.
Personnel Records — Based on LPA staff records review, staff 1 (Mandated Reporter, MMR, education requirements, LIC508, LIC9052, LIC9108 and 2023 flu declination), staff 2 (Mandated Reporter) and staff 4 (TB) are missing required Child Care Licensing forms. The licensee did not comply with the section cited above in [count] out of which poses/posed a potential health, safety or personal rights risk to persons in care.
