Personal Rights — Based on interviews and documents reviewed as noted on LIC809, Staff #1 innapropiately handled child #1 by lifting the child by the wrist while the child was resisting and upset. This posed an immediate risk to the health, safety, or personal rights to children in care.

Personal Rights — Based on interviews and document review, Child #1 sustained an injury while in care that required medical attention. Per interviews, child #1 was hanging from the soccer goal nets and the goal posts flipped and landed on the child. Per interviews, the goal posts were not secured and anchored to the ground during the incident, which posed an immediate risk to the health, safety, and personal rights of children in care.

During today's inspection LPA gathered information that staff left a child unattented for approximately 2 minutes which poses a potential health, safety or personal rights risk to persons in care.
