Infant Safe Sleep — Based on observation and record review, the licensee did not comply with the section cited above in that two (2) Infants were sleeping in a unlicensed room, door was closed, licensee was not conducting the 15 minute checks, licensee did not have current or past 15 minute checks, mattresses on play yards were not well fitted, and LPA observed a stuff animal inside the play yard with an infant, checks which poses an immediate health, safety or personal rights risk to persons in care.
