Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above regarding four large storage cabinets are not mounted to the wall for earthquake safety, hole in the wall with cable wire is accessible to children, and playground broken toy mounted to the wall accessible to children which poses/posed a potential health, safety or personal rights risk to persons in care.
