LPA observed floors to have dirt, debris, and food particles, walls, doors, cabinets, and drawers with various stains and excess clutter in accessible areas of the home which poses a potential health, safety or personal rights risk to persons in care.

LPA observed spiderwebs, dog feces, loose, damaged and broken fence panels, peeling paint on patio table, peeling seat cushions on patio chairs, and collected water pooled in the backyard which poses a potential health, safety or personal rights risk to persons in care.
