Reporting Requirements — Interview conducted with licensee. Licensee stated incident occurred 2-2-26 and did not report this to the Palmdale office. UIR is regarding Child #1 hitting Child #2 with the fist on the side of the face causing a bruise. This poses a health and safety to the children in care.
