Mandated reporter training — Based on records reviewed and interview, two (2) out of six (6) staff files had expired Mandated Reporter Certificates. The licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.

Personnel Records — Site Supervisor Yasmin Juarez was not present during today's inspection and had personnel records locked and Site Supervisor did not have access to it. Based on records reviewed and interview, the staff files were not readily available for review. Staff files were missing MMR/Tdap (2 out of 6 files) and one LIC503, which poses/posed a potential health, safety or personal rights risk to persons in care.
