Teacher Qualifications and Duties — Based on observation, interview and record review, the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care. LPA Garcia observed Adriana Macias providing care and supervision to 5 children without meeting the minimum qualifications.

Child's Records — Based on observation, interview, record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. Upon review of children's files, it was revealed that the LIC 627 Consent for Emergency Medical Treatment, was not on file.
