Personal Rights — Per statements obtained, there was an incident which occurred in January of 2024, where Staff 1 was observed grabbing Child 1's forearm and pulling Child 1 close to their face and yelling at Child 1. This incident was observed inside of a classroom. There was a second incident which occurred in May of 2024, where Staff 1 was observed grabbing the face of Child 1 and yelling at Child 1. The second incident took place in the play yard. Multiple staff also made disclosures of observing Staff 2 handle Child 2 and Child 3 in a rough manner and yell at Child #2. Per statements obtained, In March of 2024, there was an incident where Staff 2 was observed grabbing the face of Child 2 and yelling at Child 2, telling Child 2 to "Stop Crying". On multiple occasions, Staff 2 was also observed handling Child 3 in a rough manner. This poses a potential Health or Safety, or Personal Rights risk to persons in care.
The date by which the licensee must file CCLD's Proof of Correction form, certifying under penalty of perjury that the deficiency was corrected on or before that date. Filing it does not prevent CCLD from making a follow-up visit to verify.

Reporting Requirements — LPA discovered that facility staff failed to report multiple incidents involving Staff members 1 and 2 and Children 1 - 3 to Community Care Licensing (CCL). The incidents occurred between January - May 2024 and involved Staff handling children in a rough manner and Staff yelling at day care children. Per Director / Rita Greenspan, the incidents listed above were not reported and Unusual Incident/Injury Reports (LIC 624) were not submitted to CCL. This poses a potential Health or Safety, or Personal Rights risk to persons in care.
