Personal Rights — Based on interviews and demonstration, Licensee uses inappropriate discipline with the day care children. LPA uses improper time-out by placing the children in the corner. Family home — home free of safety hazards — Based on LPA's observation, the licensee did not comply with the section cited above in that LPA observed two electrical outlets without the plate covered. LPA observed two holes in the wall. This is unsafe and poses a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on LPA Wheatley record review, the licensee did not comply with the section cited above in that the licensee has not been documenting the infants sleeping/napping every 15 minute on regular basis. LPA observed a notebook whereby the lciensee keeps notes and has noted some infant napping times. This poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on LPA Wheatley observation and record review, the licensee did not comply with the section cited above in that the two assistants do not have proof of Mandated Reporter Training.
