Reporting a child's injury or suspected abuse — The Department learned that on 04/09/26, Staff #1 was arrested for DUI. The licensee did not report the incident to the Department which poses a potential health and safety to children in care.

LPA's observation of the picnic table furnishing. the table does not have back support and not age appropriate for child #1. Child#1's feet do not reach the floor and table. This poses potential health and safety risk to children in care.

(C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child." This requirement was not met by...
