Mandated reporter training — Based on LPA's observation and record review, the licensee did not comply with the section cited above in that all the staff have expired Mandated Reporter Training which expired in April / May 2025. This poses a potential health, safety or personal rights risk to persons in care.

Health-Related Services — Based on LPA's observation and record review, the licensee did not comply with the section cited above in that Child #1 has an Epi-pen and other medication on the premises which does not have permission by the parent or authorized representative.
