Responsibility for Providing Care and Supervision — Based on LPA observations conducted during today's visit the facility did not comply with the section cited above.. During today's inspection LPAs observed 16 children in care while S2 did not have visual supervision of all children in care on multiple occasions, which poses/posed an immediate health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on LPA observations, interview and record review, the facility did not comply with the section cited above. During today's inspections LPA's observed 34 children in care with two adult staff members providing care and supervision, which poses/posed an immediate health, safety or personal rights risk to persons in care.
