Buildings and Grounds — The licensee did not comply with the section cited above in Playground equipment is in need of repair, LPA observed a play house located in the rear play yard that needs to be repaired or removed. The surface of the outdoor activity space is in need of repair. LPA observed the rear play yard to have areas along the walls and 2 doors that needs to be repaired due to paint chippings and a large wooden board along the wall that needs to be repaired due to damaged bottom portion. which poses/posed a potential health, safety or personal rights risk to persons in care.
