Fixtures, Furniture, Equipment and Supplies — Based on observation, interview, record review, the licensee did not comply with the section cited above in the building physical plant having rust in bathroom #2 (one toilet and both sinks), cabinet doors missing in Destiny room, rust ring and counter sink loose in Tranquility room, storage room needs to be inacessable to children which poses/posed a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — It was revealed that the facility did not follow up with Parent #1 to confirm receipt of a message sent through the Pro Care app at approximately 10:00 AM, when Child #1 was initially injured. Parent #1 became aware of the message only after calling the facility regarding a separate issue. This delay in communication may pose a potential health and safety risk to children in care.

Personal Rights — Child #1 has a history of behavioral concerns. On this occasion, Teacher #3 did not closely monitor Child #1, which allowed Child #1 to choke Child #2. This incident poses a potential health and safety risk to children in care.
