Family home — home free of safety hazards — Based on observation service tag is dated with last inspection on July 22, 2024, the licensee did not comply with the section cited above in [1] out of [1] fire extinguisher not up to State Fire Marshall standards which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization — influenza, pertussis and measles — Staff #1 did not have proper immunization's present at the facility, records against Tuberculosis, influenza, pertussis, and measles was not observed or reviewed. Per Staff #1, needs to find immunization records.

Reporting Requirements — based on interviews conducted with Licensee admitted that she did not report incident that occurred on 07/14/24 and police came to her home 07/16/24. Licensee stated she did not report because she did not that she had to. Per Licensee, staff is qualified but but paperwork is in the garage. This poses a potential risk to the health and safety of children in care.
