Responsibility for Providing Care and Supervision — Based on interviews, observations and record review, Child #1 exited the Toddler playground emergency exit door. Minutes later, Child #1 was returned to the facility by an employee affiliated with St. Anne’s Family Services Transitional Housing Placement Program, which poses an immediate risk to the health, safey, and personal rights of children in care.
