Reporting Requirements — Based on interviews conducted the director did not report the termination of a former staff member who had inappropriate conduct with children outside of the facility. This poses a potential health and safety risk.

To receive safe, healthful and comfortable accommodations, furnishings... — Based on interviews conducted it was disclosed by S1-S3 and C1-C2 that S1 and S4 were involved in a verbal altercation in front of children. This poses a potential health and safety risk to children in care.

Buildings and Grounds — Based on observations the licensee did not comply ,with the section cited above . During the tour LPA observed black, brown , yellow stains and holes on the carpet . Water dispencer had stains and dust . LPA observed couch cushions soiled , which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA reviewed 5 satff files and observed 3 staff members were missing records of immunization , which poses/posed a potential health, safety or personal rights risk to persons in care.
