Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above in under swing does not have proper cushioning and blue ladder does not have proper fall zones, which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on the interviews and information obtained Facility did not have operable toilets for the staff and children . Children had to walk to the different facility located in the different address to use the restrooms .
