Reporting Requirements — Based on observation, the licensee did not comply with the section cited above in The Director Arleen Uryu was not present and has not been present at the facility since approximately February 2026 and November 2025 accroding to staff which poses/posed a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on observation the center did not comply with the cited above section. On August 5, 2025, LPAs observed several children playing on the outdoor cement pavement during the hot noon without their footwear. This poses a potential health and safety risk to children in care.

Responsibility for Providing Care and Supervision — Based on observation, the center did not comply with the section cited above. The center did not provide sufficient supervision, which led to the child climbing on the roughly four-foot bookshelf and jumping down. LPAs observed this during the inspection. Failing to provide adequate supervision to meet the needs of children in care poses a potential health and safety risk for them.
