Outdoor Activity Space — Based on observation, the outdoor play yard area that is shared between preschool and school-age children, does not have a shaded rest area for the children. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on review of video footage, Staff #1 was violating Child #1's personal rights by forcing the child to lay on the cot during nap time.

Fixtures, Furniture, Equipment and Supplies — During inspection, LPA observed two small blue couches in room P3 that have several stains on them. LPA also observed two small brown couches in the Pre-K room where the upholstry is ripping on the arm rests. This prevents the furniture from being sanitary. The licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
