Reporting Requirements — Based on an interview on March 9, 2026, with the Site Supervisor, it was determined that the facility did not comply with the cited section above, creating a potential risk to health, safety, or personal rights of persons in care. The facility did not report an epidemic outbreak to Community Care Licensing

Buildings and Grounds — Based on observation classroom 10 had mouse glue traps with mice droppings. Based on information gathered mice droppings were also discovered on the floor and mice were seen in classroom. This poses a potential health and safety risk to children in care.
