Infant Care Center Sleeping Equipment — Based on LPA interview and record review, A2 was instructed to place the crib in front the baby gate by the door in the nap room and stay in room 2 with the infants who were awake. This poses/posed a potential health, safety and/or personal rights risk(s) to children in care.

The facility did not ensure outdoor activity space was in a safe condition for the activities planned which poses/posed a potential health, safety and/or personal rights risk(s) to children in care.
