Responsibility for Providing Care and Supervision — Based on LPA observation and interviews, on 3/11/2026 staff did not have visual supervision of C1 and C2 resulting in C2 pushing over a bookshelf on to C1 which poses a potential risk to the health, safety, and personal rights of persons in care.

Personal Rights — Based on LPA interview, facility did not ensure bookshelfs (31”high x 12”wide x 48”long) were properly secured resulting in one bookshelf falling onto C1 which poses a potential risk to the health, safety, and personal rights of persons in care.
