Health-Related Services — Based on observation, interview and record review, the licensee did not comply with the section cited above, 5 medications were expired. 1 medication had a replacement and the expired medication was not discarded nor return to child's authorized representative which poses a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on interviews with staff, tickets are taken away from children when they do unkind to others or does not follow directions. And S2 admittedly stated she says "you can do better next time" "next time color the white spaces" to children. S3 confirmed she has heard S2 utter "you can do better next time" This is a potential risk to personal rights of children in care
