Infant Care Food Service — Based on the information gathered from the self-reported unusual incident report, observations, and interviews with three staff, it was determined that S1 fed the wrong bottle of breast milk to Child #1 (C1) that belonged to Child #2 which poses a potential health, safety or personal rights risk to the persons in care.

LPA observed large trash bins, brooms, and empty boxes, children in Toddler B use the bathroom sink to wash their hands which poses/posed a potential health, safety or personal rights risk to persons in care.
