Responsibility for Providing Care and Supervision for Infants — Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed infant changing table to not be within arms reach of the sink which poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Care Food Service — Based on observation, the licensee did not comply with the section cited above. While touring the infant room at 10:08 AM, LPA observed children bottles not labeled with the current date and one bottle was not labeled with the infants name and the current date which poses/posed a potential health, safety or personal rights risk to persons in care.
