Staff-Infant Ratio — Based on interview, staff witnessed infant room be out of ratio. This requirement was not met as evidenced by: Staff stated they were out of ratio at drop off the week of February 2-6, with 6 infants to 1 staff. LPA observed staff juggling around to cover ratios. Record Review showed facility was out of ratio between Feb 2-6, 2026. This poses a potential risk to health and safety of children in care
