In classrooms #7 LPA observed 3 out 18 parents signed children in but did not sign them out which posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on observation, interview, and record review. The licensee did not comply with the section cited above. At 2:00 pm, during staff file review and interview, staff 1 (S1) did not have their mandated reporter training certificate expired 4/19/2025, which posed a potential health, safety or personal rights risk to persons in care.
