Responsibility for Providing Care and Supervision — Based on observation, interviews, and medical records review, staff who were present at the playground did not provide adequate supervision: did not see/prevent C2 kicking C1, causing C1 to be injured. This poses a potential risk to the health, safety, and person rights of the children in care.

Mandated reporter training — Based on observation, interview, and record reviews, the licensee did not comply with the section cited above. At 1:15 pm, during staff file reviews and interview, 5 out of 5 stafff members did not have their current mandated reporter training certificate, which poses a potential health, safety or personal rights risk to persons in care.
