Personal Rights — Based on interviews and record reviews, C2 and C3 had multiple incident reports, in which, both C2 and C3 exhibited aggressive behaviors towards others for approximately 1-2 years while attending facility, and facility did not have a plan to timely address and assist C2 and C3 in managing the challenging behaviors. This posed a potential risk to the health, safety and personal rights to the children in care.
