This poses a potential risk to the health, safety and personal rights risk of the children in care.

Outdoor Activity Space — Based on observation and interview, there are cracks on the stairs of a slide in the small playground, 3 broken metal horses laying on the side planter of this playground and S1 confirmed children do play with the horses during outside time. Limitations on Capacity and Ambulatory Status — Based on interviews, P2 confirmed that C2 (8 years old) is at the facility between 2:30pm and 4pm for 2 weeks, and 5 of 5 interviewed staff confirmed that C2 was at the facility for a period of time ranging from 5 min to 2 hours.
