Responsibility for Providing Care and Supervision for Infants — Based on observations and interviews, on 3/18/2026, Child 1 (C1) was left alone without supervision at the sandbox on the playground for approximately three (3) minutes while the children were transitioning to the Orchard area outside of the facility. This poses immediate risk to the health and safety of the children in care.

Family home — in-ground pool drowning prevention requirements — Based on observation, the licensee did not comply with the section cited above, the vertical clearance from bottom to enclosure is more than 2 inches. The cover of the spa did not withstand the weight of an adult. There is no alarm in the SPA. Facility doesn't have Pool Log. These poses a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on LPA observations, inspection and interviews, there were wooden pallet in the orchard with nails sticking out of the pallet, and metal screws sticking out from metals attached to the wall.
