Mandated reporter training — Based on observation, interview, and record reviee, the licensee did not comply with the section cited above in Staff #1 (S1) did not have current Mandated Reporter training certificate was expired on 8/14/2024, which poses/posed a potential health, safety or personal rights risk to persons in care.
