During facility tour LPA was provided a Student Medication Permit which documents C1 was given medication on 9-25-2023. LPA was also provided the prescribed medication for S1. Health-Related Services — Based on the observation, the Licensee did not meet the above regulation which poses an immediate Health risk to the children in care. Napping Equipment — Based on observation and interview, the licensee did not comply with the section cited above in the LPA observed napping cots made of mesh material stacked with sleeping bags, blankets and linen exposed and touching, which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During staff file review LPA was unable to review a current Mandated Reporter certificate for S5 and S6.
