Responsibility for Providing Care and Supervision — Based on LPAs interviews and records review staff did not provide supervision to meet the children's need which posed an immediate health, safety, or personal rights risk to the children in care.

Personal Rights — Based on LPA interviews and records review, the facility did not comply with Title 22 regulation-personal rights. A child was handled in a rough manner when pulled by the arm and holding them back by the shoulders which posed a potential health, safety or personal rights risk to children in care.

Personal Rights — Based on LPAs interviews, staff taunted and threatened a child due to the child refusing to nap and not getting along with others. In addition, Staff repeatedly slammed their fists on a desk and repeatedly stated to call the child’s authorized representative.

Indoor Activity Space — Based on observation, the licensee did not comply with the section cited above which poses a potential personal rights risk to persons in care. Upon inspection, LPAs and LPM observed carpet floors that contained dirt build up.
