In conducting a physical inspection and in reviewing the facility sketch, LPA observed a middle classroom that was created by taking away space/square footage from the infant classroom. The Department did not receive updated documentation an/or authorized the alterations. This poses/posed a potential health, safety or personal rights risk to persons in care.
