Indoor Activity Space — Based on observation, the licensee did not comply with the section cited above. In conducting a physical inspection, LPA observed a classroom that was created by taking away space/square footage from the infant classroom. The classroom no longer has sufficient square footage for the licensed capacity. The Department did not receive updated documentation an/or authorized the alterations. This poses/posed a potential health, safety or personal rights risk to persons in care.
The date by which the licensee must file CCLD's Proof of Correction form, certifying under penalty of perjury that the deficiency was corrected on or before that date. Filing it does not prevent CCLD from making a follow-up visit to verify.
Alterations to Existing Buildings or New Facilities — Based on observation and record review, the licensee did not comply with the section cited above. In conducting a physical inspection and in reviewing the facility sketch, LPA observed a middle classroom that was created by taking away space/square footage from the infant classroom. The Department did not receive updated documentation an/or authorized the alterations. This poses/posed a potential health, safety or personal rights risk to persons in care.
