Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above. In reviewing children files, it was determined that child #1, did not have a Sleeping Plan (LIC 9227) on file. This poses/posed a potential health, safety or personal rights risk to persons in care. *Please note that this is a repeat violation within 12 months and a $250.00 civil penalty is being assessed*
