Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in staff one did not have physician report on file at time of inspection. This poses a potential health, safety or personal rights risk to persons in care.

Food Service — Based on observation and interview, the licensee did not comply with the section cited above in there was two expired frozen milks, one in the refridgerator and one in the freezer at the time of inspection, photos taken, milk frozen unable to smell for odor. This poses a potential health, safety or personal rights risk to persons in care.

Plan of Operation — Based on record review LPA observed that the policy for unacceptable behavior also known as the “three strike policy,” was not being followed and when C1 has behavioral issues the child is taken to the director and provided a cookie, walks etc.
