During review of staff files the LPA was unable to review immunizations for S2. Additionally, LPA was also unable to review a TB clearance for S2 or S3.

During the children’s file review the LPA review sleep logs for C1, C2, C3 that were timed for every 30 minutes. All sleep logs had set times such as 12:15PM, 12:45PM, 1:15PM, etc.
