Alterations to Existing Building or Grounds — Based on the observation the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During facility tour LPA observed off limits areas not being made inaccessible to children in care. Off limit areas have latches at the top of the doors, however, the latches were not latched. Additionally, the LPA observed one of the doors open.
The date by which the licensee must file CCLD's Proof of Correction form, certifying under penalty of perjury that the deficiency was corrected on or before that date. Filing it does not prevent CCLD from making a follow-up visit to verify.
Child's Records — Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the review of children’s records the LPA was unable to review LIC 9150 Parent Notification Additional Children in Care for C1.

Family home — admission procedures and parent rights — Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the review of children’s records the LPA was unable to review LIC 995A Notification of Parents Rights for C1 and C3.

Infant Safe Sleep — Based on the interview and record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the inspection the LPA was unable to review sleep logs for C3. Licensee stated she was informed that the sleep logs were to be completed until the child turned one.
