Child's Medical Assessments — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health to the children in care. During the children’s file review the LPA was unable to review an LIC 701 Physicians Report for C4.

101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Based on the observation and interviews, the Licensee did not meet the above in that during the investigation, it was revealed staff did not follow proper procedures during a medical emergency. This poses a potential Personal Rights risk to the children in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation, the licensee did not comply with the section cited above in a total of 5 toilets designated for preschool children are in need of repair in Rooms #2 & across room #3, two toilets have water leaks at the base of the commode & the tank lid is broken, toilet seat is deteriorating on third toilet, and rusted exposed mounting screws. In addition, the wall paint in bathroom #2 is peeling behind the commodes, which poses a potential health, safety or personal rights risk to persons in care.
